of family practice, Our comprehensive primary care services include the full spectrum of
outpatient care, including newborn and infant care, pediatric, adolescent, adult and
geriatric care, gynecological services, minor surgical procedures, and preventative
services. We also provide obstetrical services.Our Nurse Practitioner, in collaboration
with our physicians, is available to see patients with acute and chronic illnesses and
render proper treatment. |

are ill, we would like you to make an appointment so that we may evaluate your problem

| Appointments Patients are seen by appointment only.
Please call 607-431-5757 Monday-Friday 8:300 a.m. -5:00 p.m. to schedule an appointment.
Routine checkups can be scheduled up to three months in advance. If you require a same day
appointment, we encourage you to call our office early in the morning. Please provide the
medical assistant with as much information as possible when scheduling your appointment.
Every effort is made to see you at the time of your scheduled appointment, however
patients with urgent or special problems may take precedence. You may notice patients who
arrived after you being called into the exam room ahead of you. These patients are
probably seeing a nurse or nurse practitioner or may require a special set-up in an
available room. |
| Cancellations/ Missed Appointments We discourage
missed or no-show appointments. They are an inconvience to our staff as well as to other
patients. Please contact our office as soon as possble if are unable to come to an
appointment. |
| After Hours/ Weekends After hours and on weekends
and holidays, our telephones are answered by an answering service. Urgent messages will be
realyed to the physician on call by the FoxCare answering service. |
| Prescriptions and Refills Just as we cannot treat an
illness over the telephone, we cannot prescribe medications over the telephone. Long term
medications refills will be handled during regular office hours and ONLY if you are under
our care. If you need long term medication refills, have the name and/or Rx number of the
medication, the pharmacy name, and the dosage available when you call. It is most
important to call for refills early in the day, so that we may have ample time to handle
your request.
NO ROUTINE PRESCRIPTIONS WILL BE HANDLED AFTER REGULAR OFFICE HOURS OR ON A WEEKEND. |
| Fees/Insurance Payment is expected at the time of the
visit. Patients who have HMO insurance are required to pay their co-pay at the time of
service. Patients who have health insurance must present their insurance card or
other proof of insurance (including all identification numbers, covered dependents, and
effective date) each time a service is rendered.

Patients are responsible for any uncovered charges. Patients will be billed on a
monthly basis for any outstanding balance. Payment is due immediatly on receipt of billing
statement. Any unpaid balance over 60 days will be turned over for collection action. If
you are unable to pay your outstanding balance, please contact the Business Office to make
a payment plan. |
