The A.O. Fox Memorial Hospital Nursing Home is a fully accredited facility licensed by the New York State Department of Health and accredited by the Joint Commission on the Accreditation of Healthcare Organizations, a voluntary distinction reflecting our committment for providing quality care.

We believe that each resident is unique and so we work to develop a plan of care to meet each individual's needs. Our goal is to help you obtain and maitain the best possible health and level of functioning.

Each weeek, the staff meets in an interdiscplinary care conference, to review resident's medical, social, and emotional needs. You and your family will be invited to participate in these meetings to help you establish short and long terms goals.

Arrangements for admission are made through one of the nursing home social workers. A resident can only be admitted on the order of his or her physician. All residents will be admitted and treated withou regard to race, creed, color, national origin, sex, handicap, marital status, source of payment  or sexual preference.

Financial arrangements must be made prior to admission. The two most common sources of long term care payments are individual funds or Medicaid. If a resident qualifies, Medicare will pay for limited lengths of time. A coverage determination will be made upon admission.

We require that all residents, regardless of their physical or mental capabilities, arrange for someone to have power of attorney (POA), to act in their behalf should this become necessary.

We hope to involve your family in the nursing home and will be inviting them to a family conference shortly after your admission.

  • Medical/ Dental
    • The nursing home employs its own physician. ANy resident is able to use our physician's services unlees your personal physician (who must be a member of the A.O. Fox Medical Staff) agrees to care for you.
    • Dental services are provided on site
  • Nursing Care
    • Nursing Services are provided 24 hours a day by registered nurses, licensed practical nurses and geriatric care technicians.  Each resident is assessed on admission, and a care plan is written to address his or her physical and emotional needs. Each nurse is extensively trained to carry out the care plan, and to be constantly updated on safety and care techniques.
  • Rehabilitation Services
    • Physical and occupational services are available to nursing home residents, as well as speech services. Evaluations are performed on admission.
  • Nutritional Services
    • Our nutritional services are carried out under the directed of the Chief Clinical Dietician and the Registered Diet Technician. Diets meet the needs as specified by your preferences and your physician's orders.
  • Social Service
    • A social worker will work closely with you and your family during the admission process. The social worker serves as an advocate for you in the nursing home and will help you establish a full life here.
    • When you are admitted your family or your sponsor automatically becomes a member of the Family Council of the nursing home. The Council serves as an information and advocacy forum for families.
  • Pastoral Care
    • You are encouraged to maintain ties with your own church or temple. Clergy are welcome to call at any time. A chaplain is available at all times to residents, to offer counseling services, spiritual directions, and confidential visits to residents. Religious services are conducted by many different denominations.
  • Activities
    • The activities department provides a variety of recreational programs and activities. Arts and crafts materials and instruction are provided in groups and one-on-one. Movies are offered weekly, aa are Bingo, Bible Study, parites, evening entertainment, exercise groups, cooking groups, and community outings.
  • Hairdressing
    • A beautician is available each week. Your own New York State licensed beautician or barber may do you hair in your room or in the beauty shop, schedule permitting.

Visiting hours are 8:30 a.m. to 8:00 p.m. daily. We suggest visitors come after your morning care is completed. Children are always welcome, however, they must be closely supervised. You are  welcome to go on outings with family members if you are able to do so.

On special occassions, such as family birthdays, we welcome family and friend to plan celebrations with you at the nursing home. The Family Room is available for these gatherings. This room offers a private space for families use at any time.
