The Discharge Planning Service assists those patients who may need additional services, such as how to take medicine, change dressings, or use medical equipment. We can help arrange for life skills help, and if necessary, help you find a special nursing facility that's right for you..
The Discharge Planning Team

Doctors, nurses, social workers, and other healthcare professionals work as a team at A.O. Fox Hospital to provide you with quality care, and to answer all your questions. You and your family have an important role in discharge planning. We depend on you to tell us about your health, home, family, interests, and insurance coverage to so we can help you decide what you will need when you leve the hospital.

 

Things To Consider

Things about your life may change either temporarily or permanently, due to your injury or illness. Your nurse or Dischage Planner may talk with you about any of the following:

  • a special diet
  • bedrest (and for how long)
  • medicine and supplies
  • physical and occupational therapy
  • assistance with eating, going to the bathroom, bathing or dressing
  • obtaining and using medical equipment (and borrowing if possible)
  • your ability to care for other family members
  • learnng to do special treatments (such as changing dressings)

You may advised about whether you should:

  • do cooking, cleaning, laundry and other chores
  • drive your car
  • drink alcohol and use tobacco
  • enjoy your favorite hobby or sport
Self Assessment Questions
  • Do you live alone or with others?
  • Do you have a phone?
  • Will you need to climb stairs?
  • Could you live on one level of your home?
  • Are the doorways and hallways difficult to pass through?
  • Do you have carpeting? Scatter Rugs?
  • Do you think you will need some kind of help after your hospital stay?
  • Did you have a home care agency helping you before you came to the hospital?
  • What transportation do you need for going home and getting to your doctor's office or clinic?
