life. The Diabetes Management Program at the FoxCare Center is dedicated to helping people
cope with and control this disease." |
| The Diabetes Management Program at FoxCare Center follows the
national standards as endorsed by the American Diabetes Association. for a Diabetes
Self-Management Education Program. It is open to both insulin dependent and non-insulin
dependent adult clients. |

| The Sessions The program, coordinated by a certified
diabetes educator, includes two group sessions and individual counseling sessions. All
sessions are presented by a Registered Dietitian and/or Registered Nurse.
Group Session I (3 hours)
- Overview of Diabetes
- Relationship amoung nutrition, exercise, medications, and blood glucose levels.
- Prevention, detection, and treatment of acute complications.
Group Session II (3 hours)
- Foot, skin, and dental care
- The importance of support from family and friends.
- Prevention, detection, and treatment of chronic complications.
- Behavior change strategies, goal setting, risk factor reduction, and problem solving.

| Individual Counseling Sessions Appointments are
scheduled for two, one-on-one nutritional counseling sessions and as needed for glucometer
(glucose testing) instruction and insulin administration. |
Program Requirements and Registration
A written referral from a primary healthcare provider must be sent to FoxCare Education,
Ste. 300, One FoxCare Drive, Oneonta, NY 13820. Written notification of program completion
will be sent to your health care provider.Fees may be covered all or in part by your
medical insurance. Ask your insurance provider for details. As of July 1, 1998, Medicare
will cover diabetes education for all enrollees with diabetes.

| Diabetes Support
Group The Oneonta Diabetes Support Group meets the last Thursday of each month
at 7 p.m. in the Levine Conference Room of A.O. Fox Hospital, One Norton Ave., Oneonta.
