| The Adult Day Center is a program for aging, frail and
physically or mentally challenged individuals. The Adult Day Center is a medical-model
program, offering nursing supervision, treatment, testing, and therapy programs, all set
in friendly, social environment. It is open Monday through Friday, 9:00 a.m. to 3:00 p.m.. |

| Meeting Your Needs The Adult Day Care Center can
accommodate the needs of both patients and caregivers. Our facility is totally handicapped
accessible. We can perform diagnostic tests, offer exercise or therapy sessions,
administer medications, and provide many other medical services. We can also make
arrangements for transportation or patient counseling. Our hours can usually be adjusted
to meet your schedule. You can even come to the center on a day-to-day basis - no
commitment necessary.

| Eligibility Any individual with a chronic health
condition is eligible for the program. We're able to accommodate wheelchairs, incontinent
clients, and clients requiring special medications, treatments, and lab tests. We're also
able to facilitate the needs of clients with partial memory loss and confusion. |

| Fees Our program is fully licensed by the NYS
Department of Health and is Medicaid reimbursable. Private pay clients are also
accepted. |
